Personal and Reservation Information: By signing below I confirm that my personal information including name and address are correct and current. Additionally 1 confirm the nightly room rate, arrival and departure dates are correct. 

Non-Smoking Policy: I understand that all accommodations at the Resorts are non-smoking. A $500 non refundable smoking fee will be assessed if found in violation. 

Valet Parking: I understand that a lee for Overnight Valet Parking will be assessed. 

Early Departure: I understand that if I decide to depart early, the deposit obtained at time of booking is non-refundable, and any nights thereafter will be billed in full up t0 the last day of the stay. 

Proper Attire and Beach Towels: I understand that Proper Resort attire is required while inside the resort facilities including but not limited to shoes, shirts, and cover ups. Pool and beach towels will be provided for the use in these areas, a $15 per towel fee will be assessed if towels are removed from the pool and beach areas. 

Personal Belongings: Acqualina Resort and Spa on the Beach (the "Resort") maintains in-room safes for the safekeeping your personal possessions. I understand that the Resort will not be liable or responsible for loss or damage to my personal possessions, regardless of whether they are lodged for safekeeping, beyond the extent provided by Florida Law. 

Fitness Center Agreement: By signing below I agree to abide by and follow the Rules and Regulations with respect to the Fitness Facility at the Resort. I further understand and acknowledge that, physical exercise can be strenuous and subject to risk of serious injury, which can be increased by the improper use of equipment and/or machinery. The Resort urges to obtain a physical examination from your physician prior to the use of any exercise equipment or attendance in any exercise class and to properly hydrate during all physical activity. I agree that if I engage in any physical exercise or activity, or use any amenity, exercise equipment or machinery on the premises or off premises of tJ1c Resort, I do so entirely at my own risk. I agree that I voluntarily participate in any activities and the use of the facilities and premises and assume all risks of injury, illness, or deatJ1 or the loss of any personal property in connection with same. This disclaimer of liability includes, without limitation, all injuries that may occur as a result of: (a) use of any amenities or equipment/machinery in the Fitness Facility and/or Resort and participation in any activity, class, program, personal training, group training, instruction and/or otherwise at the Resort's premises, (b) the malfunctioning of any equipment, and (c) slipping or falling while on the Resort's premises, including adjacent sidewalks and parking areas, including injuries resulting from the failure by any other guest, resident, or Resort employee or agent to properly secure or re-stack any equipment. To the extent permitted by law, this disclaimer of liability includes any liability resulting from tJ1e negligence of the employees, invitees, vendors, or agents of the Resort. In recognition of the possible dangers connected with any physical activity and the use of the Fitness Facility, I hereby knowingly and voluntarily waive any cause of action of any kind whatsoever arising as the result of such activity from which any liability may or could accrue to Acqualina, and its respective officers, agents, employees, instructors or assigns.

Authorization of Charges: I agree that my responsibility with my bill is not waived and to be held personally liable in the event that any third party (person, company, or association) fails to pay for any part of the amount of the charges. I authorize all charges for my stay to be charged to the credit card I provided. I am financially responsible for any damages incurred to the guest room and I will be provided a derailed report and summary of me charges.

COVID19
The wellbeing and safety of our guests and employees remains our highest priority. 
Miami-Dade County’s existing Emergency Order 20-20 requires the usage of Face Coverings/Masks when in public. To be compliant with the Emergency Order, Acqualina Resort and Residences is mandating that Face Coverings/Masks are to be worn at all times when traversing throughout the property; including the pool and beach areas. The Face Covering/Mask must be worn fully covering nose and mouth. When seated at our outlets the facial mask must remain in place unless consuming food and beverage, or sunbathing. Masks are required to be worn upon standing or walking through our outlets and public spaces.  
As stipulated in the Miami Dade County Emergency Order 20-20, we require social distancing for all patrons on resort premise, maintaining at minimum 6 feet of space whenever possible during any interaction. 
I, and all guests accompanying, agree to adhere to the Acqualina Resort and Residences Face Mask policy and Social Distancing policy. We understand that a breach of these policies will result in the revocation of access to the Fitness Center, Spa Facilities, Pools and Beach areas for the remainder of your stay.
COVID-19 is an extremely contagious disease that can lead to severe illness and death.  According to the Centers for Disease Control and Prevention, senior citizens and guests with underlying medical conditions are especially vulnerable. Acqualina Management, LLC, d/b/a Acqualina Resort & Residences on the Beach (“Acqualina”) has put in place strict, preventative measures to reduce the spread of COVID-19 throughout the Hotel; however, Acqualina cannot guarantee that you and/or your child(ren) will not become infected with COVID-19.  You understand and hereby acknowledge the contagious nature of COVID-19 and voluntarily assume and accept all risks that you and/or your child(ren) may be exposed to or infected by COVID-19 while visiting Acqualina. 
You, and on behalf of your child(ren), hereby agree to release, covenant not to sue, discharge, and hold harmless Acqualina and its employees, agents, servants, representatives, contractors, affiliates, members, directors, principals, and shareholders (“Acqualina Parties”) of and from any and all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating to any personal injury, illness, permanent disability, and/or death (“Claims”) that you or your child(ren) may experience or incur in connection with visiting Acqualina.  I understand and acknowledge this release includes any Claims based on the actions, omissions, or negligence of the Acqualina Parties, whether a COVID-19 infection occurs before, during, or after my, or my child(ren)’s, visit to Acqualina Resort and Residences.
